I typically recommend a hybrid model -- a small centralized team for tooling and standards with most engineers embedded in product teams -- because it balances consistency with agility. I optimize for reducing single points of failure in the skills matrix and ensuring QA ratios match the actual quality load, not industry averages. Trade-offs include the management complexity of dotted-line reporting, but the payoff is a QA organization that scales with the company."
